• AIDS

• Post-traumatic stress disorder (PTSD)

• Amyotrophic lateral sclerosis (ALS)

• Crohn’s disease

• Parkinson’s disease

• Multiple sclerosis (MS)

• Medical conditions of the same kind or class as or comparable to those above

• A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification

• Chronic nonmalignant pain caused by a qualifying medical condition or that originates from a qualifying medical condition and persists beyond the usual course of that qualifying medical condition.

2.  Visit your Doctor. You will be added to the "Patient Registry"

    (Florida's list of patients, who can legally purchase medical

    marijuana), once your Doctor can qualify you

3.  Fill out the appropriate application

4.  Using the address found on the application, send your

     completed application to the state, along with:

​

• A copy of your Florida drivers license, Florida identification card, or other proof of residency

• A $75 check or money order (application fee) made out to the Florida Department of Health

• An approved passport-type color photograph 2"x2"  in size, must have been taken within 90 days prior to filing your application.
